INSURANCE & PRIVATE PAY

Know your options before you begin

Cost and coverage matter. Connection Haven provides both insurance-based and private-pay options so you can understand the path that fits you before scheduling care.

IN-NETWORK INSURANCE

Using insurance?

Connection Haven is currently in network with:

Aetna  •  UnitedHealthcare  •  Anthem  •  Cigna  •  Blue Cross Blue Shield

Insurance coverage and out-of-pocket costs vary by plan. Please verify your benefits before your first appointment.

HOW INSURANCE SCHEDULING WORKS

Headway handles the insurance path

For insurance-based therapy, use Headway to review coverage information and continue with scheduling. Headway is the portal Connection Haven uses for insurance-related appointments.

Before scheduling: confirm your plan benefits, deductible, copay or coinsurance, and whether the service you need is covered.

PRIVATE PAY

Transparent private-pay rates

Private-pay scheduling and client portal access — including therapy groups — are handled through SimplePractice.

Individual Therapy

$165

per session

Couples & Family Therapy

$175

per session

Therapy Groups

$40–$50

per session, depending on the group

Choosing private pay

Private pay can be a meaningful choice when you want therapy to be shaped by your goals, needs, and pace – not just the limits of an insurance plan. It may offer more flexibility in the focus and length of care, and can be especially helpful when couples, family, or group services are not covered by insurance. We are happy to discuss whether private pay or out-of-network reimbursement may be a good fit for you.

Private-pay scheduling and client portal access, including therapy groups, are through SimplePractice.

GOOD FAITH ESTIMATE

Your right to a Good Faith Estimate

You have the right to receive a Good Faith Estimate explaining how much your medical and mental health care will cost. Under the law, health care providers must give clients who do not have insurance or who are not using insurance an estimate of expected charges for services.

You may receive this estimate in writing at least one business day before your service, or you may request one before you schedule. If you receive a bill that is at least $400 more than your Good Faith Estimate, you may dispute the bill.

For questions or more information, visit www.cms.gov/nosurprises or call 1-800-633-4227.
